COURSE DESCRIPTION

This course focuses on the care of an older patient with Alzheimer’s disease and support for caregivers of such patients. In-depth information about Alzheimer’s disease is provided. Therapeutic approaches and community-based interventions appropriate throughout the course of the disease are explored. Other topics addressed include home safety, substance abuse in the older patient, legal issues in advance care planning, medication review and reconciliation, polypharmacy and the role of palliative care in dementia.

Prerequisite: FlourishCare: Integrated Person-Centered Geriatric Primary Care
